Form 5:14: Initiate List Instructions

The Initiate List Form is to be prepared by each chapter to report new members. There are four categories of information that are to be provided:

1. Dates of chapter tapping and initiation ceremonies and number of new members.

2. Summary of payment for new members:

a. National dues at $40 per collegiate member
b. Honorary dues at $40 each (includes pin)
c. Conference assessment at $350 per chapter
d. Pins - optional - at $7.50 each

3. Address to which membership certificates and pins are to be sent. If the chapter has a permanent address (e.g., post office box, campus office, or advisor's office), it should be reported. If there is no permanent chapter address, a name and address which will be in effect during the summer should be given.

4. Initiate information: All information should be typed as a Word document and sent via email.

a. Full name of each member
b. Home (permanent) address
c. Home (permanent) phone
d. Academic major - please do not use abbreviations

The outgoing chapter membership selection officer should email this list to the Executive Secretary as a word document by June 1. Send to: ex_sec@yahoo.com. Payment should be sent as single check for all fees: dues, convention assessment, pin orders (optional) and scholarship contribution.
